You must take the survey to be eligible for the prize drawing.

All survey responses are confidential and collected by an independent, third-party research firm. Your identity will not be associated with your responses.

Entering to win a prize is easy—simply fill out the form below for each location you have reviewed. Find your unique identifier number in the email UNT sent to your university email account in October.
